MEDICAL FORM

Click here to download the camper medical form.  This form must be completed entirely and is due no later than July 25th.  You do not have to send in the medical form along with the registration information.
 
Please note that the camper must have a complete physical examination by their physician within one calendar year of the start date of camp.  If your physician's office provides a print out of all of the requried information, you may attach that to the health form to avoid having to re-write everything but please ensure that there is a physician's signature and date of last physical exam on the form. 

Campers are not allowed to participate in camp without a completed medical form on file.

CHECKLIST

The checklist contains information regarding what to bring to camp, highlights of activities for the week, camp store information, directions, etc.  This checklist is e-mailed to you along with the confirmation letter once your registration information has been received and process.  If you misplace the form, you may download a copy of it here.
